Completing and sending in the Plant Configuration Form is the first step to becoming a participant in Base ORAP. The Form is a basic overview of your plant(s) structure and contact information. The information on this sheet also provides SPS with the correct data submission contact as well as Quarterly Report recipient information.
To the left you will find both a .pdf version of the Plant Configuration Form as well as a Microsoft Word version. You have the ability to download the Word version and enter your information directly on the form. After you have completed the information, please return the form to Debbie Mays at debbie.mays@spsinc.com. Please reference Plant Configuration Form in the subject line.
